The Centre int\u00e9gr\u00e9 de sant\u00e9 et de services sociaux (CISSS) de l'Abitibi-T\u00e9miscamingue, in collaboration with its many partners (family group medicine, pharmacies, community organizations, etc.) offers many services to persons at end of life and to their loved ones, while living in their home to their admission into a facility.<\/p>\n

At home, professionals assigned to palliative care from the home support team evaluate the needs, implement the services and make the appropriate referrals. The individual can benefit from many home care services (nursing care, health support, social services, occupational therapist, etc.) based on their physical condition and the progression of the disease. The team accompanies the person who remains at home until the very end, if that is what they wish. Whether at home or in a hospice, the person and their loved ones will be accompanied during the whole process.<\/p>\n

The clientele<\/h3>\n

Any individual who is in the palliative care phase aged 0 to 100 residing on the CISSS-AT territory regardless of the medical condition which led them to seek palliative care (cancer, COPD, Alzheimer, etc.).<\/p>\n
