Chronic diseases
A chronic disease is a health condition that persists for a long time, for example, diabetes, heart and lung diseases, arthritis, etc. Chronic diseases are attributable to a combination of causes that affect the individual throughout their lifetime. They include:
- Cardiovascular diseases;
- Diabetes;
- Hypertension;
- Musculoskeletal diseases (allows for body movement);
- Neurodegenerative diseases such as Alzheimer’s, Parkinson’s and multiple sclerosis;
- Mental health problems;
- Cancer.
Chronic disease self-management care program
This program consists of a series of six free meetings that allow the participants to acquire the skills and confidence to better manage their day-to-day health condition.
